The version without description can be found in Bromjunaar Sanctuary in Labyrinthian, on the ground next to a skeleton.

Recipe

Crafted at a Tanning Rack with the following materials:

• Wooden Mask
• 1 Firewood.

Notes

• Requires Amulets of Skyrim.
• Contrary to all other rings and amulets which are crafted from Dragon Priest masks, all versions are unenchanted. The description is a broad hint that you have to equip the mask for its enchantment to work.
• You can craft left- from right-handed rings at a Tanning Rack and change them back the same way.
• The enchanted ring (right handed) can be re-crafted back into the Wooden Mask at a Tanning Rack.
• The museum display accepts all four versions.
