Requires Artifacts of Skyrim.
The approximate location is shown on Artifact Map - VI sold by Urag gro-Shub at the College of Winterhold.
The helm respawns.
If Immersive College of Winterhold is installed, the helm may fall through the island geometry, requiring noclip to retrieve. A patch for this can be found here (the patch relocates the helm next to the chest in the Skytemple Ruins). It can also fall off the mountain and land on the beach below. In that case, Schneider's Sextant or the Locate Relic power can help you finding it.
